10/01/2013 SLIP OR FALL OF PERSON DYS AWY FRM WRK & RESTRCTD ACT 175d lost, 463d restricted Fell 15 feet to the ground while on the cat walk of jaw crusher directing backhoe operator.
12/08/2011 POWERED HAULAGE FATALITY Fatal Employee was placing removable legs on a transfer conveyor. Conveyor was held in place by a loader. Removable leg fell and struck employee in the face, fatally injuring the employee.
