03/22/2022 HANDTOOLS (NONPOWERED) NO DYS AWY FRM WRK,NO RSTR ACT None Employee was buckling two suction hoses together. EE was using a 12 inch crescent wrench as leverage. It slipped off the buckle and hit EE in the right side of EE's forehead.
09/16/2021 HANDTOOLS (NONPOWERED) NO DYS AWY FRM WRK,NO RSTR ACT None Employee was changing out a turbo on a Hitachi Excavator. The ratchet (hand held tool) EE was using broke causing the hand to strike the turbo flange lacerating the pinky finger.
