04/21/2015 FALL OF ROOF OR BACK DYS AWY FRM WRK & RESTRCTD ACT 1d lost, 7d restricted Employee, roof bolter, exited equipment and was struck by a rock off the bolted back. He received three stitches and bruises. Fifteen minute was notified and investigated by MSHA.
