03/19/2009 IGNITION OR EXPLOSION OF GAS OR DUST DAYS AWAY FROM WORK ONLY 30d lost Victim was using a torch to remove a valve bank from a roof bolter. The torch accidentally contacted an unseen hydraulic hose and caused oil to spray out and ignite burning his left arm and face.
