10/18/2009 HANDLING OF MATERIALS DAYS AWAY FROM WORK ONLY 14d lost EE was taking down belt hardware on X cut 5 CU 4 belt, EE was taking out rails from inside of belt hangers one slid out and landed on top of her left foot. Bruise to top of EE's foot.
