10/03/2018 HANDLING OF MATERIALS DAYS AWAY FROM WORK ONLY 2d lost EE was cleaning material from under the hopper with a shovel. EE scooped up some material and turned to toss the material off the shovel and felt a pull in EE's back.

06/03/2016 INUNDATION FATALITY Fatal A haul unit was being loaded by a track hoe and a slush pond dam broke and the slush came down on top of them burying them.
06/03/2016 INUNDATION FATALITY Fatal A track hoe was loading a haul unit with gravel to take to wash plant and the dam on a slush pond broke and covered up both the tack hoe and the haul unit with the workers in them.
