11/16/2009 MACHINERY DAYS AWAY FROM WORK ONLY 37d lost EE was moving the miner from the #7 to the #6 entry. The miner went over a hump in the bottom, this caused the back of the miner to raise into the top.
08/13/2009 POWERED HAULAGE DAYS AWAY FROM WORK ONLY 16d lost While tramming shuttle car from miner to dump, shuttle car dropped in a hole. The deck of car kicked up a rock, hitting EE in the eye.
12/02/2008 INUNDATION ACCIDENT ONLY None While mining in the # 5 entry of the 001 section, cut into old works. The size of the cut through approx. 2' X 4'. The mine is blowing ventilation therefore no ventilation issues with cut through.
