04/06/2017 MACHINERY DAYS AWAY FROM WORK ONLY 308d lost EE was bolting in #3 heading around 1:15 pm. EE was attempting to push a roof bolt into the top when ee's bolt wrench kicked out of drill pot and struck ee's left hand.
