09/18/2025 POWERED HAULAGE DAYS AWAY FROM WORK ONLY 7d lost The scoop guy was pushing a skid in to the belt line. The skid caught a roll of belt pushing it sideways into the employee who was in a crosscut, bruising EE's hip.
02/12/2025 SLIP OR FALL OF PERSON DAYS AWAY FROM WORK ONLY 58d lost EE was exiting a scoop and twisted knee. EE was excused from work on 3/11/2025, and will be getting meniscus repaired in right knee.
12/05/2024 SLIP OR FALL OF PERSON DAYS AWAY FROM WORK ONLY 37d lost EE slipped on ice getting out of EE's vehicle and hit EE's head. This resulted in a concussion. EE was excused from work 12/10/2024.
11/19/2024 MACHINERY NO DYS AWY FRM WRK,NO RSTR ACT None While installing a bolt, EE pinched pinky finger between the plate and drill head. Which resulted in stitches.
10/28/2024 HANDLING OF MATERIALS DAYS AWAY FROM WORK ONLY 74d lost EE was trying to remove pin from the low-tail, when the tail moved back. EE's finger got pinched between the pin and the top surface of the tail. Which resulted in stitches.
