03/08/2012 FALL OF ROOF OR BACK ACCIDENT ONLY None 1-intersection outby spad #286. Roof fall - #1 entry - return approximately 4800' from surface. Fall 25' long, 18' wide, 5'-6' high. Did not effect air way. Did not effect travelway.
