08/30/2011 HANDLING OF MATERIALS NO DYS AWY FRM WRK,NO RSTR ACT None Employee using pitot tube to measure air in a mine fan. The pitot tube got too far into the fan, the employee lost his grip on it and the pitot tube whipped around and smacked the employee's hand.
07/27/2011 HOISTING ACCIDENT ONLY None Elevator down due to car brake hanging when car coming to landing - car repaired on 7/28/11 at approximately 1:00pm.
07/26/2011 HOISTING ACCIDENT ONLY None Hoist brake car would not operate safely due to coil/contactor being bad on braking system. Contactor replaced and car was operational at approximately 6:00 am on 7/27/11.
