10/26/2007 HANDLING OF MATERIALS DYS AWY FRM WRK & RESTRCTD ACT 12d lost, 20d restricted Getting out of truck, twisted back wrong and has pain in back. There were discrepancies in the doctors notes, so we waited to hear back from work comp, that is why the time difference for the report.
