11/20/2008 SLIP OR FALL OF PERSON DAYS RESTRICTED ACTIVITY ONLY 145d restricted EE was pulling belt with channel locks and changed locks, slipped and EE fell into rib and hurt right shoulder. EE had to have surgery on 12/8/08. Returned to work on light duty 12/9/08.
