04/23/2011 HANDLING OF MATERIALS NO DYS AWY FRM WRK,NO RSTR ACT None EE was dragging a 18 H.P. Pump that weighs approx. 300lbs. off # 6 panel approx. 800 feet. EE hurt the lower part of his stomach.
04/16/2011 FALL OF ROOF OR BACK ACCIDENT ONLY None In the No.4 Entry (primary escapeway) between x-cuts No.4 and No.5 going underground from the surface, there was a rock fall measuring 15' wide, 8' long, and 4 1/2' thick.
03/09/2011 FALL OF ROOF OR BACK ACCIDENT ONLY None During the weekly exam of air counts a rock fall was discovered on 3/9/11 at 6:25am in the #4 intake air course. Approx 20' inby the mine fan. The rock fall is approx 10' wide X 8' long X 4' high.
