09/29/2008 FALL OF ROOF OR BACK DAYS AWAY FROM WORK ONLY 2d lost EE was mucking the #5 belt at the overcast. A piece of draw rock fell and struck him on the right shoulder and the arm. This resulted in pain, bruising and swelling of the right arm.
08/28/2008 HANDLING OF MATERIALS DAYS AWAY FROM WORK ONLY 2d lost EE was replacing a tire on the std car on section 008. The tire slipped, resulting in EE straining his back (upper) and his neck. Pain in back and neck.
