09/29/2016 STRIKING OR BUMPING NO DYS AWY FRM WRK,NO RSTR ACT None While walking up a flight of stairs the EE struck head on a beam jamming neck. On 11/11/16 we were informed that EE was receiving chiropractic treatments for the injury.
