08/08/2006 HANDLING OF MATERIALS DAYS AWAY FROM WORK ONLY 159d lost Unit #3 - Employee was installing 8' roof bolt. The bolt did not tighten up. Employee was trying to pull the bolt back out of the hole and off pod. Felt a pop in right shoulder.
