04/19/2006 HOISTING ACCIDENT ONLY None Bad top door landing switch. Elevator was repaired and put back into service at 2:52 a.m.
04/12/2006 HOISTING ACCIDENT ONLY None The elevator doors would not open or close properly. The doors were adjusted and elevator put back in service at 09:02 am.
04/04/2006 HOISTING ACCIDENT ONLY None Broken cable pulley for Main Portal Elevator doors. Elevator was repaired, inspected and put back into service at 10:25 a.m. on 4/5/06.
04/03/2006 HOISTING ACCIDENT ONLY None After mine examination and power restored (power company had power off last shift) found electrical problem with hoist. Blown fuse was replaced and hoist was put back in operation at 8:45 a.m.
03/31/2006 HOISTING ACCIDENT ONLY None Due to high winds, Main Portal Facility lost power causing loss of elevator and slope. Power was re-established at 12:35 a.m. on 4/1/06 and elevator and slope were back in service.
